Job summary

CVS Health is seeking a Case Manager Registered Nurse to deliver in-home assessments and coordinate care for members in Union County and surrounding areas. This field-based role requires strong clinical judgment and excellent communication to support member health.

Qualifications

• Associate's degree or nursing diploma required.
• 3+ years of clinical practice in hospital or other settings.
• Active NJ RN license required.
• NJ Choice certification must be completed as a contingency.

Responsibilities

• Conduct face-to-face home visits using assessment tools.
• Evaluate medical needs of members in MLTSS and D-SNP programs.
• Develop, implement and promote health service strategies and policies.
• Collaborate with providers and care teams to create and authorize care plans.
• Advocate for members to ensure safe discharge with appropriate services.
• Document accurately in the member's electronic health record.
• Mentor new hires once proficiency is attained.
