Job summary

Chapters Health System seeks an RN, Care Coordinator to assess patient/family needs and coordinate the Plan of Care with the IDG, providing palliative and supportive care. Qualifications

• Current RN license in the state where employed.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Active BLS for healthcare professionals.
• Ability to visit Participant in their homes for assessments.
• Ability to effectively communicate in English (verbal and written).
• Must have valid driver’s license and automobile insurance per company policy.

Responsibilities

• Provides and manages direct care as part of IDT.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patient/family about disease, palliative care, and safety practices.
• Reports changes in patient condition to IDT and physicians.
• Participates in IDT meetings and coordinates referrals.
