Job summary

Qualifications

• Active and unrestricted RN license in NJ.
• 3+ years of clinical practice experience.
• Must reside in Camden County or surrounding area.

Responsibilities

• Conduct in-person visits and assessments in members' homes across NJ.
• Coordinate multidisciplinary care plans and advocate for safe discharges.
• Document accurately in the member's EHR and communicate with care teams.
• Travel approximately 75% to member homes and clinics.
