ForzaCare is seeking a Field Medical Case Manager to guide injured workers through recovery and return-to-work journeys. You will coordinate care among medical providers, employers, and insurers and travel to appointments as needed.

The ideal candidate holds an active RN license or CRC certification, with workers' compensation experience strongly preferred and a valid driver's license for on-site visits.

Qualifikationen

• Active RN license or CRC certification required.
• Prior workers' compensation case management experience preferred.
• Must have valid driver's license and ability to travel to appointments.

Aufgaben

• Coordinate care between medical providers, employers, and insurers.
• Attend appointments with injured workers at their place of employment or other sites.
• Develop, document and monitor individualized recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders and ensure compliance.
