ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return-to-work. You will coordinate among medical providers, employers, and insurers to ensure timely, transparent care management.

This role is ideal for licensed nurses or certified rehabilitation counselors, with preference for workers’ compensation experience. Travel to appointments is required as needed.

Qualifikationen

• Active RN or CRC license with related degree.
• CCM, CIRS or other case management credentials preferred.
• Must meet state licensure requirements and be willing to travel.

Aufgaben

• Coordinate care among medical providers, employers, insurers, and injured workers.
• Attend appointments at worksites or clinics as needed.
• Develop and monitor tailored recovery and return-to-work plans.
• Provide ongoing progress reports to clients and stakeholders.
• Ensure compliance with customer and regulatory requirements.
• Foster strong relationships with clients, providers and team members.
