Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate recovery and return‑to‑work plans for injured workers. You will serve as the central point of coordination among medical providers, employers, and insurers to ensure timely, transparent care management.

This role is ideal for licensed nurses (RN) or certified rehabilitation counselors (CRC). Prior workers’ compensation experience is strongly preferred, with travel to appointments as needed.

Responsibilities

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments with injured workers, including at employer sites or workplaces.
• Develop, document, and monitor individualized recovery goals and return‑to‑work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
