Job summary

ForzaCare in the United States is hiring a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will act as the central coordinator among medical providers, employers, and insurance carriers to ensure timely, transparent care management.

This role is ideal for licensed nurses (RN) or certified rehabilitation counselors (CRC). Prior workers' compensation experience is strongly preferred, and you must be willing to travel to appointments.

Qualifications

• Active RN or CRC license with degree is required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure requirements.
• Prior experience in workers' compensation case management preferred.
• Valid driver''s license and ability to travel to appointments.

Responsibilities

• Coordinate care among medical providers, employers, and injured workers.
• Attend appointments with injured workers, potentially at their workplace.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide regular progress reports to clients and stakeholders.
• Ensure case management work meets compliance requirements.
• Build strong relationships with clients, providers, and team members.
