ForzaCare is seeking a Field Medical Case Manager to coordinate recovery and return-to-work for injured workers. You will connect workers, providers, employers, and insurers to ensure timely, transparent, and effective care management.

Ideal candidates are licensed nurses or certified rehabilitation counselors. While workers' compensation experience is strongly preferred, related backgrounds are valued.

Qualifikationen

• Active RN or CRC license required.
• CCM, CIRS or other case management credentials preferred.
• Must comply with state licensure requirements.
• Prior workers' compensation case management experience preferred.
• Valid driver's license and ability to travel to appointments.

Aufgaben

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments with injured workers, possibly at their place of employment.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
