Job summary

ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting them with providers, employers, and insurers to ensure timely, transparent case management. The role suits licensed nurses or CRCs, with prior workers' compensation experience preferred and a valid drivers license for travel.

Qualifications

• Active RN or CRC license with corresponding college degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.
• Valid drivers license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at workplaces or locations relevant to the injured worker and care plan.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal teams.
