Job summary

ForzaCare is seeking a Field Medical Case Manager to guide injured workers through recovery and a safe return-to-work process. You will coordinate with medical providers, employers, and insurers to ensure timely, transparent care management.

Ideal candidates are licensed nurses or certified rehabilitation counselors, with prior workers' compensation experience highly valued. Travel to appointments may be required, and you will document progress and communicate with stakeholders consistently.

Qualifications

• Active RN or CRC license with associated college degree is required.
• Additional certifications such as CCM or CIRS are preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including at their place of employment.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
