Job summary

ForzaCare is hiring a Field Medical Case Manager to coordinate recovery and return‑to‑work plans for injured workers. You will serve as the central link among medical providers, employers, and insurers to ensure timely, transparent care management.

Ideal candidates are licensed nurses or CRCs; prior workers’ compensation experience is strongly preferred. Travel to patient sites and appointments is expected as part of the role.

Qualifications

• Active RN or CRC license required.
• CCM or CIRS certifications preferred.
• Travel to appointments may be required.

Responsibilities

• Coordinate care among medical providers, employers, and carriers.
• Attend appointments with injured workers at work sites or clinics.
• Develop and monitor individualized recovery goals and return‑to‑work plans.
• Provide consistent progress reports to clients and stakeholders.
• Ensure case management work meets compliance and customer needs.
• Maintain strong relationships with clients, providers and team members.
