ForzaCare is seeking a Field Medical Case Manager to guide injured workers through recovery and return-to-work processes. You will serve as the central coordinator among injured workers, medical providers, employers, and insurers to ensure timely, transparent, and effective care management.

The role is ideal for licensed nurses or certified rehabilitation counselors, with workers' compensation experience preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.

• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
