Job summary

ForzaCare, a nurse-led care coordination firm, seeks a Field Medical Case Manager to guide injured workers through recovery and return-to-work.

You will coordinate with medical providers, employers, and insurers, attend appointments, set recovery goals, and deliver timely progress reports while upholding safety and compliance.

This role requires an active RN or CRC license; CCM or CIRS credentials are a plus, and workers' compensation experience is strongly preferred.

Qualifications

• Active RN or CRC license with associated college degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at the workplace or other locations as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
