Job summary

ForzaCare is seeking a Field Medical Case Manager to guide injured workers through recovery and return-to-work pathways. You will coordinate with medical providers, employers, and insurance carriers to ensure timely, compliant care management and clear progress reporting.

The role requires active RN or CRC licensure, with CCM/CIRS credentials preferred. Must be willing to travel to appointments and job sites, supporting a patient-centric care approach in workers' compensation cases.

Qualifications

• Active RN or CRC license required.
• State licensure compliance required.
• Valid driver's license and ability to travel to appointments.
• Prior workers' compensation case management experience preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to worksites.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
