Job summary

ForzaCare is seeking a Field Medical Case Manager in Nashville to coordinate care for injured workers, collaborating with providers, employers, and insurers to ensure timely return-to-work plans. This role emphasizes central coordination, documentation, and transparent communication to stakeholders.

Licensed RNs or CRCs with applicable credentials are preferred, and prior workers' compensation experience is valued.

Qualifications

• Active RN or CRC license with a college degree is required.
• CCM, CIRS or other case management credentials are preferred.
• Must comply with state licensure requirements.
• Prior experience in workers' compensation case management is strongly preferred.
• Valid driver's license with ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments, including visits to employers and workers at their worksite.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal teams.
