Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will coordinate with medical providers, employers, and insurers to ensure timely, transparent care management and seamless communication.

This role is ideal for licensed nurses or certified rehab counselors. Prior workers' compensation case management is strongly preferred, and willingness to travel to appointments is required.

Qualifications

• Active NJ Compact RN license required.
• NY RN license preferred.
• Prior workers' compensation case management experience strongly preferred.
• CCM, CIRS or other case management credentials preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments at workplace or with injured workers as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
