Job summary

ForzaCare in Memphis, TN seeks a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will connect the worker, medical providers, employers and insurers to ensure timely, transparent, and effective care management.

Ideal candidates are licensed RNs or Certified Rehabilitation Counselors. Prior workers' compensation case management is strongly preferred, though related experience is valued.

Qualifications

• Active RN or CRC license required.
• CCM, CIRS or other case management credentials preferred.
• Willing to travel to appointments with reliable transportation and valid driver's license.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers and injured workers.
• Attend appointments at employers' sites or injured workers' locations.
• Develop, document and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers and internal team members.
