Job summary

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) in a field-based role that primarily operates in the field with travels to client sites and occasional office attendance. Location is flexible within designated counties in Ohio, with hybrid work options according to policy.

Responsibilities include leading person-centered planning, conducting assessments, and developing comprehensive care plans while coordinating with multidisciplinary teams and the member’s support network.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency or equivalent background.
• Strong preference for case management experience with older adults or individuals with disabilities.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Manage service coordination for a designated caseload in LTSS programs.
• Collaborate with individuals to lead Person Centered Planning, documenting preferences, needs and goals.
• Conduct assessments and develop comprehensive Person Centered Support Plans (PCSP).
• Develop backup plans and work with Medical Directors and interdisciplinary teams to establish integrated care plans.
• Engage the individual's support network and oversee management of physical and behavioral health and LTSS needs.
• Submit utilization/authorization requests and document alignment with care plans.
• Report critical incidents to appropriate internal and external parties and participate in audits and appeals.
