Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) for a field-based role requiring travel across multiple Florida counties. The candidate should reside within commuting distance to designated counties and be willing to travel as needed to client sites and meetings.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• LTSS, case management, social work or hospital discharge planning experience preferred.

Responsibilities

• The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs, including assessments and development of a comprehensive Person Centered Support Plan (PCSP).
• Interfacing with Medical Directors and participating in interdisciplinary care rounds to support integrated care plans and management of PH/BH/LTSS needs.
• Engages the member’s circle of support and ensures documentation of goals and preferences.
• Identifies members who would benefit from alternative levels of service or other waiver programs.
• Submits utilization/authorization requests and reports critical incidents to appropriate agencies; participates in appeals and state audits.
