Qualifikationen

• BA/BS degree required.
• Minimum of 2 years experience with a social work agency.

Aufgaben

• Perform face-to-face program assessments using predefined questions.
• Coordinate with clinical management and interdisciplinary teams to support member care.
• Manage non-clinical needs for members with chronic illnesses to ensure efficient LTSS use.
• Document member goals in PCSP with care team.
• Identify members who may benefit from alternative service levels or waivers.
• Mentor or preceptor for new staff and contribute to process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to appropriate authorities and participate in audits.
