Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members across the care continuum. You will assess goals and barriers in-home or in community settings, develop individualized plans, and connect members with LTSS, HCBS, and community supports to promote independence and better health outcomes.

You will travel regionally to meet members and collaborate with interdisciplinary teams, providers, and family caregivers.

Qualifications

• Bachelor’s degree in social sciences, social work, human services, or a related field.
• Two years of experience working with Medicare/Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One or more years of professional experience with custodial and transitional care.
• Experience using electronic information systems, including EMRs.

Responsibilities

• Meet members in the field at home, NF, or member location to assess goals, needs, and barriers.
• Develop care plans and coordinate LTSS, HCBS, and community resources.
• Coordinate with care teams and providers to promote member independence and well‑being.
• Educate members on Medicaid eligibility and navigate LTSS and related services.
• Travel within the region to engage with members, families, and care partners.
