Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work in the field, meeting with members at home or NF facilities to develop personalized care plans and coordinate long-term services and supports. The position requires Illinois residency, a social sciences degree, and experience with Medicare/Medicaid, LTSS, and EMR systems.

Qualifications

• Must reside in the state of Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two years of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One or more years of professional experience working with custodial and transitional care.
• Experience using a variety of electronic information applications/software programs including electronic medical records.
• Ability to travel in the region to meet face-to-face with members and/or their families, community partners and care teams.

Responsibilities

• Meet members in the field at home, Nursing Facility (NF), or location of the member's choice. Assess goals, needs, and barriers; connect members with services.
• Provide LTSS support, advocate for member care, and assess return-to-community options.
• Develop and modify Individual Service Plans with care team involvement.
• Support members navigating LTSS and environmental/social needs; coordinate with HCBS providers and payers.
• Facilitate access to Home and Community-Based Services to promote independence.
