Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members in validating care plans and coordinating LTSS. You will visit members in their homes or facilities, assess goals and barriers, and link them to housing, services, and supports for better independence and health outcomes.

You will work in a field-driven role with 75-90% travel, primarily remote, and occasional Humana office visits.

Qualifications

Must reside in the state of Illinois.
Bachelor's degree in social sciences, social work, human services, or a related field.
Two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, or managed care organizations.
One (1) or more years of professional experience working with members in custodial and transitional care.
Experience using a variety of electronic information applications/software including electronic medical records.
Ability to travel in the region to meet face-to-face with members and/or their families, community partners and care teams.

Responsibilities

Meet members in the field at home, NF, or location of the member's choice, assess goals and needs, and develop plans of care.
Coordinate LTSS services, advocate for member care, and connect with health services and community partners.
Engage in interdisciplinary care planning with care teams and informally coach caregivers.
Assist in navigating LTSS, HCBS, and other services regardless of funding sources.
Educate members to maintain Medicaid eligibility and support entry of Level of Care assessments.
