Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members in Illinois, focusing on personalized care plans, service coordination, and improving health outcomes. Responsibilities include building relationships with facility staff, advocating for member care, and coordinating with care teams to access community resources.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• 2+ years of experience with Medicare/Medicaid recipients, long-term care services, HCBS, or managed care.
• 1+ year of experience with custodial and transitional care.
• Experience using a variety of electronic information applications/software including EMR.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet with members in the field to assess goals, needs, and barriers and develop plans of care.
• Coordinate services and connect members with LTSS providers and community partners.
• Advocate for member care, including access to behavioral health services.
• Support members and caregivers using an interdisciplinary approach and person-centered thinking.
• Educate members to maintain Medicaid eligibility and assist with annual Level of Care assessments.
