Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members, creating personalized care plans, coordinating services, and empowering better health outcomes and greater independence.

You will meet members in the field at home, nursing facilities, or other locations, assess goals, and connect them with LTSS providers and community resources to promote well-being. Requires IL residency and travel.

Qualifications

• Must reside in Illinois.
• Two (2) years of experience with Medicare/Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One (1) or more years of professional experience with custodial and transitional care.
• Experience with electronic information applications/software, including EMR.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet with members in the field (home, NF, or member's location) to assess goals, needs, and barriers; develop plans of care and coordinate LTSS and community resources.
• Provide LTSS support focusing on HRSNs, psychosocial support, and alignment of LTSS with member service needs.
• Build relationships with facility staff to advocate for member care and access to services; assess readiness to return to the community.
• Develop and modify Individual Service Plans with care team involvement; navigate LTSS and related social needs.
• Educate members to maintain Medicaid eligibility and assist with Level of Care assessments.

Education

Bachelor's degree in social sciences, social work, human services, or a related field
