Job summary

Humana is seeking a Transition/Custodial Prevention Specialist in Illinois to work one-on-one with members, creating personalized care plans and coordinating services to improve health outcomes and independence. You will visit members at home, Nursing Facilities, or the location of their choice, assessing goals and barriers to tailor LTSS and HCBS supports.

The role requires 75–90% field travel, collaboration with care teams, and navigating Medicaid eligibility.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field is required.
• Two years’ experience with Medicare/Medicaid recipients or HCBS/managed care.
• One+ year in custodial or transitional care.
• Proficiency with electronic information applications including EMR.

Responsibilities

• Provide specialized LTSS support focusing on health-related social needs.
• Build relationships with facility staff and advocate for member care.
• Develop and modify plans of care with the care team.
• Navigate LTSS and related services to promote independence.
