Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members in coordinating LTSS and HCBS. You will meet clients in the home, NF, or a location of their choice and develop person-centered care plans to promote independence and better health outcomes.

You will travel regionally, collaborate with care teams, and support Medicaid eligibility education while navigating diverse funding sources. A bachelor’s degree and LTSS experience are required.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care.
• One or more years of experience with custodial and transitional care.
• Experience using electronic information applications/EMR systems.
• Ability to travel regionally to meet members and families.

Responsibilities

• Provide specialized support for LTSS with focus on health-related social needs and ensuring services meet member needs.
• Support members residing in nursing facilities by liaising with facility staff and advocating for care.
• Contact members to assess goals, evaluate resources, and develop plans of care with LTSS providers.
• Develop and modify Individual Service Plans involving care team members.
• Assist members navigating LTSS and related environmental and social needs.
• Educate members on Medicaid eligibility and coordinate with payer sources and providers.
