Job summary

Humana is seeking a Transition/Custodial Prevention Specialist in Illinois to work with members at home, NF, or location of their choice. You will assess goals, coordinate LTSS, and connect members with HCBS and community resources to improve health outcomes and independence.

In this field role, you will travel regionally (75–90%), collaborate with care teams, and document plans of care while promoting person-centered approaches and Medicaid eligibility education.

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two years of experience with Medicare/Medicaid, LTSS, HCBS, or managed care.
• One year or more of professional custodial and transitional care experience.
• Experience with electronic information systems including EMRs.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet members in the home, NF, or location of choice to assess goals and barriers.
• Coordinate LTSS, HCBS, and other services to address member needs.
• Develop care plans with the care team and informal supports.
• Support member navigation of LTSS and related social needs.
• Educate members about Medicaid eligibility and program options.
