Job summary

Humana is seeking a Transition/Custodial Prevention Specialist in Illinois to work one-on-one with members to create personalized care plans, coordinate LTSS, and empower them to achieve better health outcomes and greater independence.

This field role requires 75–90% travel to meet members and caregivers, collaborate with care teams, and connect people with community resources and Medicaid supports for durable, person-centered care.

Qualifications

• Bachelor’s degree in social sciences or related field.
• At least 2 years with Medicare/Medicaid, HCBS, or managed care.
• At least 1 year in custodial or transitional care.
• Experience using EMR software.

Responsibilities

• Provide LTSS support focused on health-related social needs and psychosocial care.
• Collaborate with nursing facility staff to advocate for member care.
• Meet members in home or community to set goals and plans.
• Develop care plans with the care team and relevant providers.
• Educate members about Medicaid eligibility and access to services.
• Assist with annual Level of Care assessments.
