Job summary

Humana is seeking a Transition/Custodial Prevention Specialist in Illinois to work with members in homes, NF, or their chosen location. You’ll assess goals, develop plans of care, coordinate LTSS, and connect with community resources to improve health outcomes and independence.

You will travel 75–90% to meet members and families, collaborate with care teams, and navigate Medicaid eligibility while promoting person-centered supports and access to HCBS.

Qualifications

• Must reside in Illinois and have a Bachelor’s degree in a related field.
• Two years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care.
• One or more years in custodial/transitional care with members.
• Experience using electronic information applications including EMRs.

Responsibilities

• Assess member goals, needs, and barriers in the field or at home.
• Develop plans of care and coordinate LTSS, HCBS and related services.
• Connect members with community resources and payer providers.
• Engage interdisciplinary teams to support member independence.
• Educate members on Medicaid eligibility and navigate LTSS processes.
• Travel regionally to meet members and families for care planning.
