Job summary

Qualifications

• Must hold LPN/LVN, LSW, LCSW, LMSW or RN license per state law.
• Minimum 2 years working with individuals with chronic illnesses in a service coordinator role.
• Current, unrestricted license in applicable state(s) required.

Responsibilities

• Assist the RN in identifying high-risk members and collecting clinical data.
• Help determine if members need alternative care or waiver programs.
• Provide all collected information to the RN for care plan development and monitoring.
• Coordinate care for members with chronic illnesses with the health team.
• Apply clinical guidelines to decision making while avoiding direct nursing judgments.
