Job summary

Elevance Health is seeking a LTSS Service Coordinator – Clinician to operate primarily in the field, visiting client sites with occasional office attendance. The role centers on coordinating physical, behavioral health, social services, and long term supports for members.

The position requires licensure and experience in chronic illness coordination, with a focus on trauma-informed, person-centered care. Travel to Columbus, OH and surrounding areas is expected, within commuting distance policy.

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or licensed alternative as allowed by state law; 2+ years caring for individuals with chronic illnesses or disabilities.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Assists in identifying members for high risk complications and obtain clinical data.
• Assists in identifying members who could benefit from alternative level of care or waiver programs.
• Collaborates with RN to develop, monitor and revise the member's care plan.
• Coordinates care for members with chronic illnesses with the health care team to ensure cost effective utilization of benefits.
• Uses clinical guidelines and current disease knowledge to support member care decisions.
