Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to operate in the field, traveling to client sites in Johnson County IN and Shelby County IN with occasional office attendance. You will work under the supervision of an RN to support LTSS care coordination including assessments of health, behavioral health, and social needs.

The role requires LPN/LVN or social work licensure (LSW/LCSW/LMSW) with at least 2 years' experience with chronic illnesses or disabilities, and may require MS

Qualifications

• LPN/LVN, LSW, LCSW or LMSW license as required by state law.
• Minimum 2 years experience with chronic illnesses or disabilities in a service/coordinator role.
• Unrestricted state license in applicable state(s) required.
• State-specific certification may be required by law or contract.

Responsibilities

• Assist RN in identifying high-risk members and data collection.
• Identify candidates who may need alternative care levels or waivers.
• Support care plan development and ongoing monitoring with the RN.
• Coordinate care with members, health team and services to ensure efficient use of benefits.
