Job summary

Humana Inc. is seeking a Transition/Custodial Prevention Specialist in Illinois to work in the field, visiting members in homes, nursing facilities, or other locations of choice.

You will assess goals and needs, develop personalized care plans, and coordinate LTSS with community partners to promote independence and health outcomes. The role requires travel (75–90%), with possible onsite meetings at Humana's Schaumburg, IL office.

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or related field.
• Two years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care.
• One year or more of experience in custodial and transitional care.
• Experience using electronic information applications/software including EMR.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet members in home, NF, or chosen location to assess goals and plan care.
• Coordinate LTSS and connect members to community resources.
• Develop care plans with applicable care team members.
• Advocate with facility staff for member care and behavioral health services.
• Identify LTSS providers and community partnerships for services.
• Educate members on Medicaid eligibility.
• Assist with annual Level of Care assessments where needed.
• Collaborate with interdisciplinary teams to support member goals.
