Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work remotely in Illinois with substantial regional travel. You will assess members in homes or facilities and coordinate LTSS and HCBS to promote independence and better health outcomes.

You will collaborate with care teams, focus on Medicaid eligibility, and develop personalized plans. This role requires field visits and strong organizational and interpersonal skills.

Qualifications

• Must reside in the state of Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two (2) years of experience working with Medicare and Medicaid recipients, LTSS, HCBS, or managed care.
• One (1)+ years of experience with custodial and transitional care.
• Experience with electronic information applications/software, including EMR.

Responsibilities

• Meet members in the field at home, NF, or member location; assess goals and priorities and develop plans of care.
• Coordinate LTSS, HCBS, and community services through interdisciplinary teams.
• Navigate Medicaid eligibility and assist with entry of Level of Care assessments.
• Develop and modify Individual Service Plans with care team involvement.
• Travel regionally to meet members, families, and partners.
