Job summary

Pacific Health Group is seeking a field-based Housing Stability Services professional to support Medi-Cal members with housing navigation, tenancy support, and deposit assistance in Monterey County. The role emphasizes independent fieldwork, collaboration with providers, and comprehensive care coordination.

You will conduct assessments, develop individualized care plans, coordinate services, and ensure timely documentation while navigating housing resources and healthcare integration for

Qualifications

• Strong understanding of the housing system and local resources
• 3–5 years in case management, social services, or healthcare
• Experience with Medi-Cal, CalAIM, and Community Supports (preferred)
• Excellent communication, empathy, and cultural competence
• Proficient with high-volume documentation and real-time data entry
• Ability to document in real-time and manage member data accurately

Responsibilities

• Support members experiencing homelessness or at risk of homelessness by providing Housing Transition Navigation Services
• Coordinate Housing Tenancy & Sustaining Services with ongoing case management and landlord engagement
• Assist members with Housing Deposits and required documentation
• Collaborate with housing authorities, landlords, shelters, and community organizations
• Travel locally within Monterey County and surrounding areas
• Build relationships with community partners to expand housing resources
• Represent Pacific Health Group in community outreach and events
• Maintain accurate and timely documentation complying with Medi-Cal and CalAIM
