Job summary

ICL seeks a Case Manager – Supported Housing to empower individuals living in independent apartments. You will coordinate medical, psychosocial, financial, and vocational services, delivering field-based support to maximize independent living.

Under supervision, you’ll assess needs, plan services, and provide crisis intervention when required, while advocating for clients and collaborating with community providers. Travel to residences is essential.

Qualifications

• Bachelor's degree and relevant direct service experience as described.
• Experience delivering services to individuals with mental disabilities or similar human services.
• Ability to obtain authorization to work in the U.S. and travel to client residences.

Responsibilities

• Travel to consumer residences to provide supportive services on a scheduled basis.
• Counsel individuals or groups to meet service needs and goals.
• Maintain case records, assessments, and service plans in regulatory compliance.
• Provide crisis assessment and intervention as needed.
• Assist consumers in accessing vocational, educational, and community supports.
• Plan and coordinate social and recreational activities and trainings.
