Job summary

ICL is seeking a Case Manager – Supported Housing in New York. The role supports individuals living in independent apartments by coordinating access to medical, psychosocial, financial, vocational, and other essential services.

Travel to client residences is required to deliver supportive services and crisis intervention as needed. The position requires a Bachelor’s degree with relevant experience or an equivalent combination.

Qualifications

• Bachelor's degree plus two years’ direct service experience or equivalent practicum experience.
• Associate's degree plus 3 years above experience.
• Work eligibility to work in the U.S.

Responsibilities

• Travel to/visit consumers’ residences to provide supportive services on a schedule set by the program.
• Counsel consumers individually or in groups to meet service needs and goals.
• Maintain case records and measurable service plans in compliance with regulatory standards.
• Provide crisis intervention as needed and deliver services in the field to support independent living.
• Refer consumers to community providers and accompany them to appointments.
• Assist consumers in accessing vocational training, employment, and education opportunities.
• Plan and coordinate social and recreational activities.
• Perform other job-related duties as assigned.
