Job summary

Institute for Community Living Inc. is seeking a Case Manager to support individuals living in independent apartments in the community. Under supervision, you will coordinate access to medical, psychosocial, financial and vocational services and provide crisis intervention as needed.

The role involves visiting client residences, documenting assessments and plans, and promoting recovery and independent living through individualized services and advocacy.

Qualifications

• Direct human services experience with individuals with mental disabilities or related human services required.
• Crisis intervention skills and ability to provide field-based support.
• Experience delivering services in community settings and coordinating access to services.

Responsibilities

• Travel to client residences to provide supportive services.
• Counsel clients individually or in groups to meet goals.
• Maintain case records, service plans, and documentation per regulatory standards.
• Coordinate access to medical, psychosocial, financial, vocational services.
