Job summary

ICL in New York City seeks a Case Manager for Supported Housing to assist individuals with mental health needs in securing stable housing and coordinating medical, psychosocial, financial, and vocational services. You will deliver crisis intervention and support independence in the community.

Under Program Director supervision, you will travel to client residences, document plans, advocate for clients, and participate in trainings to ensure safe, quality care and recovery-oriented supports.

Qualifications

• Bachelor's degree plus two years’ experience providing direct services to individuals with mental disabilities or other relevant human service experience.
• Alternative: bachelor's degree with practicum and one year experience; associate's degree with 3 years above experience.

Responsibilities

• Travel to/visit consumers’ residences to provide supportive services on a schedule established by the program; counsel consumers individually or in groups to meet service needs and goals
• Assist consumers in modifying attitudes and patterns of behavior by increasing understanding of self and others
• Secure and evaluate information related to medical, psychological, and social factors; complete and maintain assessments, service plans, treatment records, and documentation
• Maintain accurate, up-to-date case records and measurable service plans in compliance with regulatory standards
• Provide supportive counseling (individual and group) and utilize evidence-based approaches such as cognitive-behavioral techniques
• Obtain, develop, and communicate resource information and connect consumers to appropriate community supports
