Job summary

Enlyte is seeking a Field Case Manager in the Sandy Spring, GA area. This full-time, hybrid/field role requires regular local travel to attend in-person patient appointments and coordinate care with physicians, employers, and patients.

You will develop and implement treatment plans aimed at safe, cost-effective recovery and return to work, while leveraging medical resources and case management best practices.

Qualifications

• Associates or Bachelor's degree in nursing or related field is required.
• 2+ years of clinical practice preferred; workers’ compensation experience preferred.
• Active RN license in good standing; URAC-recognized CCM/CDMS/CRC/CRRN or COHN/COHN-S/RN-BC/ACM/CMC certifications preferred.
• Reliable transportation and ability to travel to in-person appointments.
• Proficiency with Excel, Word, and Outlook; strong written communication skills.

Responsibilities

• Demonstrate knowledge, skills, and competency in case management standards of practice.
• Collaborate with treating physicians/providers and employers to create and implement treatment plans tailored to an individual patient.
• Work with employers and physicians to modify job duties to facilitate early return to work.
• Independently manage workload, including prioritizing cases and deciding how best to manage cases effectively.
• Attend injured worker appointments when appropriate and prepare status updates for customers.
