Job summary

Enlyte is seeking a Field Case Manager in the Douglasville, GA area. This full-time, hybrid/field role requires regular local travel to conduct in-person patient appointments. You will work with physicians, employers, and patients to create effective treatment plans that support safe, timely return to work.

The position requires an active RN license, 2+ years of clinical experience, and strong communication and computer skills. Travel and reliable transportation are essential.

Qualifications

• RN license active in good standing.
• 2+ years clinical practice experience preferred.
• Travel to in-person appointments within assigned geography.
• URAC-recognized case management certification (CCM/CDMS/CRC/CRRN, etc.) preferred.

Responsibilities

• Develop and implement treatment plans in collaboration with physicians/providers.
• Coordinate with employers, customers, and the injured/disabled patient to facilitate return-to-work.
• Monitor progress and adjust case goals based on patient improvement.
• Manage workload and prioritize cases for efficient service delivery.
• Attend injured workers' appointments as needed and provide updates to stakeholders.
