Job summary

Enlyte is hiring a Field Case Manager in Rome, GA for a full-time hybrid/field role requiring travel to in-person appointments. The ideal candidate is an RN with 2+ years of clinical practice and applicable case management certifications.

Comprehensive benefits and a base salary range are offered. You will collaborate with physicians, employers, and patients to design and implement recovery plans, ensuring cost-effective, high-quality care and timely return to work while navigating diverse cases

Qualifications

• RN license required and in good standing.
• 2+ years clinical practice preferred; workers’ compensation experience preferred.
• URAC-recognized case management certification (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC).
• Travel to in-person appointments within assigned geography is required.
• Reliable transportation and internet access are required.

Responsibilities

• Lead and coordinate treatment plans with physicians/providers, employers, and patients.
• Assess recovery progress and determine occupational vs non-occupational factors.
• Collaborate with clinicians to tailor case management to each patient.
• Modify job duties with employers to facilitate early return to work.
• Manage workload, prioritize cases, and provide status updates to customers.
