Enlyte is seeking a Field Case Manager in the Douglasville, GA area to coordinate treatment plans with treating physicians and employers, ensuring cost-effective and effective recovery for injured workers. The role requires travel to in-person appointments and working in a hybrid/field setting.

The ideal candidate holds an RN license in good standing with 2+ years of clinical practice and URAC-recognized case-management certification options.

Qualifications

• RN license in good standing (required).
• 2+ years of clinical practice preferred.
• URAC-recognized case-management certification (CCM, CDMS, CRC, CRRN, COHN/COHN-S, RN-BC, ACM, CMAC, CMC) or equivalent.

Responsibilities

• Develop and implement treatment plans with input from clinicians, employers, and patients.
• Collaborate with physicians/providers to tailor reasonable job duties for early return to work.
• Monitor patient progress and adjust goals to achieve maximum medical recovery and optimal functioning.
• Manage workload independently, prioritizing cases and timely status updates for customers.
