Enlyte is seeking a Field Case Manager in the Santa Maria, CA area for a hybrid, full-time role requiring travel to in-person appointments. You will develop and manage treatment plans with clinicians, employers, and patients to achieve safe, cost-effective outcomes.

Qualifications include RN licensure, 2+ years of clinical practice, and URAC-certified case management. Benefits include a comprehensive package, travel reimbursement, and ongoing training.

Qualifications

• Education: Associate or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Skills: Advocate recommendations effectively with physicians, employers, and customers; work independently; basic computer skills (Excel, Word, Outlook); strong written communication.

Responsibilities

• Create and implement treatment plans with input from physicians/providers, employers, and patients.
• Collaborate with treating physicians and leverage resources to tailor plans.
• Modify job duties with employers to facilitate early return to work.
• Manage workload independently, prioritize cases, and provide status updates to customers.
• Attend in-person appointments as needed and perform other duties as assigned.
