Job summary

CVS Health Corporation's Aetna team seeks a Case Management Coordinator to support dual eligible members in Cook and surrounding counties. You will collaborate with members, providers and community partners to navigate benefits, education and care planning.

The role requires critical thinking, strong communication, and the ability to work independently from home while traveling as needed. You will monitor care plans, document outcomes and ensure quality care in line with regulatory standards.

Qualifications

• Bachelor's degree or non-licensed master level clinician in behavioral health or human services (psychology, social work, marriage and family therapy, counseling).
• Ability to travel within a designated geographic area for in-person case management activities.

Responsibilities

• Case management and discharge planning experience preferred.
• Evaluate member needs and eligibility to inform care plans.
• Coordinate and monitor care plan activities and progress.
• Collaborate with care teams to optimize quality of care and member engagement.
• Document care interactions in accordance with regulatory guidelines.
