Qualifications

• Bachelor's degree in behavioral health or human services required.
• 2 years of experience in behavioral health or social services.
• Managed care experience preferred.
• Ability to travel for in-person case management.
• Excellent analytical and problem-solving skills.
• Effective communication, organizational, and interpersonal skills.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.

Responsibilities

• Evaluate member needs and eligibility; select care plan.
• Identify high risk factors and refer to clinical care as needed.
• Coordinate and monitor care plan progress.
• Collaborate with care teams to improve quality and outcomes.
• Engage with members to promote healthy lifestyle changes.
• Document care activities per regulatory guidelines.
• Participate in team meetings and peer mentoring.
