Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing to travel 50-75% in Winnebago, Boone, Stephenson, Ogle and Jo Daviess Counties, Illinois.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members and assess needs, eligibility, and plan options.
• Identify high-risk factors and coordinate with case management.
• Coordinate and monitor assigned care plan activities and progress.
• Consult with Medical Directors and other programs to improve care quality.
• Escalate quality issues through established channels and document outcomes.
• Engage members to promote healthy lifestyle changes and adherence.
