Job summary

CVS Health is seeking a full-time field teleworker in West Virginia to support care coordination for members. The role requires residency in WV and travel of about 50% or more within the central region, including member homes and facilities.

Work operates Monday–Friday with no nights, weekends, or holidays. The specialist evaluates member needs, coordinates care plans, and collaborates with case managers and healthcare teams to improve outcomes.

Qualifications

• 2 years’ experience in behavioral health or related field equivalent to program focus.
• West Virginia resident residing in Braxton, Calhoun, Clay, Fayette, Gilmer, Kanawha, Nicholas, Roane or Webster-Regional.
• Must possess reliable transportation and be willing and able to travel in the assigned region 50% or more of the time.
• 2+ years of experience with personal computer, keyboard, mouse, multi-system navigation; and MS Office Suite applications (Outlook, Word, Excel, SharePoint, Teams)

Responsibilities

• Conduct comprehensive evaluation of referred member needs/eligibility and recommend an approach to case resolution.
• Identify high risk factors and service needs impacting outcomes and coordinate appropriate referrals.
• Coordinate and implement care plan activities and monitor progress.
• Provide coaching, information, and support to empower members in healthcare decision-making.
