Qualifications

• West Virginia resident in listed counties.
• Must travel in the assigned region 50%+; mileage reimbursed.

Responsibilities

• Conduct comprehensive evaluation of referred member's needs/eligibility and recommend an approach.
• Identify high risk factors and coordinate care planning.
• Coordinate and monitor care plan activities and progress.
• Escalate quality of care issues and support member engagement.
